Please use the following online form to submit comments, suggestions, or complaints. All complaints will be forwarded through the SPNA to the appropriate city representatives via E-mail for processing.

If you wish to remain anonymous in forwarded complaints, please still include your name and address for SPNA reference. Make a note of your wish for anonymity with your comments, and we will submit your complaints under the name of the South Park Neighborhood Association.

If you would like to print our form to write your complaints you can download it and print it by clicking HERE. You can then drop your complaint form off at the next South Park Neighborhood meeting and we will be happy to submit it for you.
